Alternate Level of Care (ALC)

Improving transitions across the entire continuum of care can reduce the number of patients designated as alternate level of care (ALC). Improvements to patient flow practices and hospital efficiency are critical, but focusing solely on these is no longer enough to improve transitions. A comprehensive, system-wide approach that includes the early identification and prevention of avoidable hospitalizations, and increased community capacity to care for people in their own homes, is also needed to ensure that people are in the “right bed” at the right time.
