The focus of our conference is Quality Matters, the health system quality framework on how to realize excellent care for all.
For Health Quality Transformation 2016, Health Quality Ontario invited the submission of abstracts focused on all six dimensions of quality care as outlined in Quality Matters: safe, effective, patient-centred, efficient, timely and equitable.

In addition, we focused on abstracts that advance quality in three areas of focus; primary care, mental health, and palliative care/end-of-life-care, at the local, regional and/or provincial level.

At HQT2016 abstracts were allocated to oral presentations in breakout sessions or as poster presentations. See a summary of abstracts here.
The top two posters with the highest mean score in each category was selected as a Poster of Distinction. The abstract with the highest mean score in each category was selected as an Abstract of Distinction.
The selection process was led by the Abstract Review Committee, which was comprised of internal and external members, including patient and family caregiver representatives.
