Note

The rate of follow-up visits per 100 discharges was lower from 2020 to 2022 than in previous years mainly because the indicator does not include virtual primary care visits, which have increased since the beginning of the COVID-19 pandemic. The results should be interpreted with caution.

In-Person Patient Follow-Up with a Family Doctor After Leaving Hospital

This indicator measures the rate of in-person follow-up visits to a family doctor within 7 days of discharge, after hospitalization for pneumonia, diabetes, stroke, gastrointestinal disease, congestive heart failure, chronic obstructive pulmonary disease, heart attack, or other cardiac conditions. It does not include virtual visits. A higher rate is better. Timely follow-up can help smooth a patient’s transition from hospital to home or community.
