A key strategy for improving surgical care is measurement and feedback to surgeons engaged in improvement and innovation. This information page is to inform you of how measurement of general surgical procedures will be conducted by Ontario Health, and offer you an opportunity to participate.

Notice of Collection: Your information is collected by Ontario Health in compliance with the Freedom of Information and Protection of Privacy Act (FIPPA) for purposes that are necessary for the proper administration of the Connecting Care Act, 2019 including the measurement of surgical care processes and outcomes, and primary care processes and outcomes. Your information will be used by Ontario Health to perform data linkages and create the quality indicators which will be shared with you. Your information may also be used for the purposes of evaluating the MyPractice program and research that complies with FIPPA.

The information that Ontario Health collects about you will be managed and protected in accordance with Ontario Health’s Privacy Policy Privacy | Ontario Health. If you have any questions, please contact practicereport@ontariohealth.ca








MyPractice Reports


What is the purpose of MyPractice Reports?

Ontario Health provides confidential MyPractice reports to physicians and surgeons to aid in quality improvement. These reports provide personalized data and suggested ideas of improvement for physicians and surgeons to support practice and quality improvement across the province. Some examples are the reports for physicians and surgeons working in hospital, primary care or long-term care. What data is involved?

MyPractice reports provide cross-sectional, longitudinal, and comparator data relevant to physicians and surgeons. These are personalized data specific to the physician/surgeon and include different aspects of their practice and may include data on a physician’s/surgeon’s prescribing patterns, patients’ demographics, case mix, and other quality metrics. Information may be provided at the practice, regional and/or provincial levels, and no patient-level data are reported.

Ontario Health uses personal health information (PHI) data it collects from a number of different data sources under its authority as a Prescribed Entity under the Personal Health Information Protection Act (PHIPA). The data sources are identified in the publication acknowledgement section. Data is linked and aggregated (i.e. no patient level information) by Ontario Health for the purposes of developing the MyPractice reports.

Your confidential reports will only be provided to you and will not be shared with others, including other agencies or physicians or surgeon groups.
