Designing an Ontario Patient Safety Learning System


At the request of the Ministry of Health and Long-Term Care, Health Quality Ontario gathered a working group of experts to develop recommendations on the design of an Ontario Patient Safety Learning System. The goal of this work is:

“To establish a mechanism where hospitals must share what they have learned from their investigations of critical incidents, and their recommendations to prevent future incidents with each other.” (Quality of Care Information Protection Act Review Committee recommendation)

The working group’s report addresses the intended goals of reporting, the kind of information that should be reported and the processes that could support sharing learning across the province.

The report has been submitted to the Ministry for consideration as Health Quality Ontario works in partnership with select hospitals to design such a mechanism for collecting and sharing this information.
