Opening Remarks
Deputy Minister Mr. Saäd Rafi
Saäd Rafi is a results-driven leader with a track record of innovation. Deputy Minister of the Ontario Ministry of Health and Long-Term Care since February 2010, he has been spearheading the largest transformation of the health care system since the 1950s. He is widely respected for his commitment to a culture of quality and accountability, and promoting a person-centred system that delivers sustainable, high quality health care. Mr. Rafi has previously served in fields as diverse as infrastructure, energy, transportation and public/private partnerships. He holds a Bachelor of Arts and a Master of Arts, in Public Administration, from Carleton University.

Keynote Address
Mr. Jeffrey Simpson
Jeffrey Simpson, The Globe and Mail's national affairs columnist, has won all three of Canada's leading literary prizes -- the Governor-General's award for non-fiction book writing, the National Magazine Award for political writing, and the National Newspaper Award for column writing (twice). He has also won the Hyman Solomon Award for excellence in public policy journalism. In January, 2000, he became an Officer of the Order of Canada.
Mr. Simpson has published seven books. Chronic Condition, his latest, is an incisive analysis of the past, present, and future of Canadian health care and recently won the Donner Prize for Best Public Policy book by a Canadian.

Morning Breakout Sessions
This session explored best practices in care coordination. Through the lens of medication reconciliation, this session examined the progress of an individual through the health system and between sectors. Organizations from across Ontario shared their successful approaches to improved care coordination and the tools and techniques that led to measurable improvement in medication safety and patient experiences.

This session focused on evidence and its role in the development of quality-based procedures in the hospital setting, as well as how they will inform the Ministry of Health & Long-Term Care’s health system funding strategies, with a specific focus on the novel approach to quality-based procedures developed by Health Quality Ontario. This session also reviewed the framework used to guide the implementation of these quality-based procedures.

Afternoon Keynote
Dr. Vincent Lam
Dr. Vincent Lam is an emergency physician at Toronto East General Hospital, and a writer. Dr. Lam's first book, Bloodletting and Miraculous Cures, won the 2006 Scotiabank Giller Prize, and was adapted for television. Dr. Lam co-authored The Flu Pandemic And You, a non-fiction guide to influenza pandemics, which received a Special Recognition Award by the American Medical Writers' Association in 2007. Dr. Lam's biography of Tommy Douglas was a Globe and Mail ‘Best Book’ in 2011.
The Headmaster's Wager, Dr. Lam's recent novel, was a finalist for the 2012 Governor General's Prize, and was shortlisted for the 2013 Commonwealth Book Prize.

This interactive session brought together thought leaders from both health and non-health organizations to engage in a stimulating discussion on how to achieve transformational change in a complex environment. This session focused on the strategic, motivational, and effective leadership necessary to achieve the goal of a high-performing health system.

This session highlighted the approach that Guelph is taking to effectively leverage community resources in order to accelerate sustainable change and improve connections to services. The Guelph Community Wellbeing Initiative (CWI) started the discussion on social determinants of health. By linking to the Guelph Health Link and by drawing on the experiences of the Prince Albert Community Mobilization Project, these combined initiatives aim to provide more coordinated, person-centred care, fostering collaboration between health and non-health sectors. Presenters shared how this collaborative approach to health delivery leads to the more efficient use of community resources to reduce demands on the health care and other system and achieve better outcomes for individuals, families and the community.

Dr. Andreas Laupacis
Dr. Laupacis is a general internist and Executive Director of the Li Ka Shing Knowledge Institute of St. Michael’s Hospital. He is a Professor in the Faculty of Medicine at the University of Toronto. His research interests are in health services research and public engagement. He holds a Canada Research Chair in Health Policy and Citizen Engagement. He has served on numerous academic and governmental committees. Other current activities include Chair of the Board for Health Quality Ontario, a Board member of Cancer Care Ontario and the chief editor of www.healthydebate.ca.
